For Senior Advantage Members
The product and services described below are neither offered nor guaranteed under our contract with the Medicare program. In addition, they are not subject to the Medicare appeals process. Any disputes regarding this product and services may be subject to the Kaiser Foundation Health Plan of the Northwest grievance process.
Current Senior Advantage members can enroll in the Kaiser Permanente Healthy Smile Dental Discount Program for Senior Advantage members.
Card holders receive an average of 20 percent to 50 percent off of almost all General Dentists’ and Specialists’ standard fees for oral exams, cleanings, teeth whitening, orthodontics (braces), root canal therapy, gum disease treatment, and many more services.

How It Works
It’s easy! When you receive your Healthy Smile ID card, simply choose a participating dental office and make an appointment. Present your Healthy Smile ID card when you arrive for your appointment. The dental office will charge you the appropriate fee for services included in the program.

Reminder: you must be a current Northwest Region Kaiser Permanente Senior Advantage member to enroll in Healthy Smile Dental Discount Program for Senior Advantage members.
Application Effective Date:
Your Healthy Smile Dental Discount Program for Senior Advantage members will be effective the date that your application is processed. This date will be on your Healthy Smile Program ID card that will be mailed to you.
This program is not insurance. It is a discount program. You are responsible for the full cost of any health care services purchased. You will receive discounts for dental services at certain dental providers who have contracted with the program. Card holders have the right to cancel registration within a 30 day period. This program does not make payments directly to dental providers.
A list of all program providers within the prospective card holder’s geographic area which includes their name, city & state, and specialty is available prior to purchase, upon request. Discounts for hospital services are not available. This program is administered by Group Dental Service of Maryland, Inc. (GDS-MD), your Discount Medical Plan Organization, 111 Rockville Pike, Suite 700, Rockville, MD 20850, 866-498-7909. The program and its administrators have no liability for providing or guaranteeing service or the quality of service rendered.
