What is the advantage of the Healthy Smile Plan?
The plan was designed to make dental care affordable for uninsured/underinsured individuals, families and groups by providing average savings of 20 percent to 50 percent off almost all providers’ standard fees. In addition, there are no benefit maximums, no waiting, and virtually no paperwork.

How much does it cost to enroll in the Healthy Smile Plan?
Annual costs to enroll are $98.00 for individuals and $150.00 for families. A family plan can include up to a total of 9 people who either share the same primary residence or students in college.

When is my plan effective?
Upon receiving your Healthy Smile ID card, you are eligible to make an appointment with any participating DenteMax or Kaiser Permanente Dental Provider and start saving!

How do I start using Healthy Smile?
As soon as you receive your ID card you may contact any of the participating providers and schedule an appointment. You will need to present your ID card at your appointment. The dental office will charge you the appropriate fee for services included in the plan. Payment is due to the provider at the time of service. From time to time, certain providers may offer products and services to the general public at prices lower than the discounted prices available through this plan. In such an event, the card holders will be charged the lowest price.
