As a Dental Choice member, you have access to more than 92,000 participating dentists nationwide, and more than 1,200 participating dentists in our service area (Northwest Oregon and Southwest Washington). This includes the dentists in our Kaiser Permanente dental offices.

Click here to find a participating provider in your area. Pick an office close to home, work, or school. You can always change dentists if you want.

Please note:

• Results from the provider search do not guarantee that a provider is a current participant in the PPO network. Before you schedule an appointment, we recommend you call the provider or Customer Care to double-check whether the provider is currently in the PPO network.
• Participating providers have agreed to file claims for you. If your dentist does not participate in our network, you may need to file a claim yourself. To get the claim form, visit the submit a claim page. When you visit a non-participating dentist, remember to take a claim form with you. Your dentist can help you fill it out.
